Abstract

Thalassemia is a hereditary disorder with high prevalence worldwide and in Indonesia, particularly beta thalassemia major, which presents the most severe clinical manifestations. Patients with beta thalassemia major require lifelong regular blood transfusions, which increase the risk of iron overload. Iron accumulation can be assessed using ferritin levels, while inflammatory status can be evaluated through the neutrophil lymphocyte ratio (NLR). This study aimed to determine the cutoff point of ferritin levels as a predictor of increased NLR in patients with beta thalassemia major. A cross-sectional design was applied with a total of 71 patients aged 0–18 years at Dr. H. Abdul Moeloek Regional Hospital, Bandar Lampung, in 2023. Data were obtained from medical records and analyzed using the receiver operating characteristic (ROC) curve. The results showed an area under the curve (AUC) value of 0.735, indicating acceptable discriminative ability. The optimal ferritin cutoff point was identified at 2907.145 ng/mL, with a balanced sensitivity and specificity. Patients with ferritin levels ≥2907.145 ng/mL had a higher risk of elevated NLR, indicating increased inflammatory response. These findings suggest that ferritin levels can serve as a predictive parameter for assessing inflammation risk in beta thalassemia major patients. This parameter may assist healthcare providers in monitoring patient conditions and improving clinical decision-making.
